Day Hospital Program
The Day Hospital Program at Miller-Dwan Rehabilitation Center fills the gap between traditional inpatient and outpatient
medical rehabilitation. It’s more intensive than outpatient therapy because it provides
multiple treatment sessions involving therapy, education and nursing care.
The
program can help bridge re-entry from a hospital into the community or help patients achieve
goals so they can remain in the community.
Each patient’s treatment and services are based on his or her unique needs. Functional
goals may include self-care; mobility; thinking and understanding; adjustment to
disability; bowel and bladder control; self medication; and speech and language.
At Miller-Dwan Rehabilitation Center at SMDC Medical Center we offer:
- Rehabilitation from a team -- therapists, nurses
and social workers -- that collaborates to help meet each patient’s individual goals.
Limited nursing assistance
- Hours are 8 a.m. to 4 p.m. Monday through Friday
but can be adjusted based on patient’s treatment plan
- Visits with rehabilitation physician, usually once
a week
- Care conferences with patients and their families
every two weeks to monitor and discuss treatment and goals
- A place for patients to rest between therapies
- Meals can be brought from home or purchased in our
cafeteria. Simple meal preparation can also be incorporated into therapy
- Education for patients and family
- Health Portfolio, which compiles health information
into a notebook designed to help a patient and caregivers manage ongoing health-care
needs and resources in their community
- Referrals to community agencies that can help the
transition back into the community
